I’ve heard this question being asked several times in the past. When there’s a problem in production, one of the first questions often is
Why did not testing find this?
To be honest, that’s fine. I don’t see that being a bad question as it’s good to consider if we should change our testing approach in some way. What I don’t like about this is though that this is as far as it often goes.
I’ve rarely seen people asking
What is wrong in our development approach?
or perhaps even better
What will we learn from this?
Because each problem in production is asking each of us look into mirror and consider how to grow as a team. As we are responsible of the quality of the product.
